1) I am aware that this event is intended, EXCLUSIVELY, for medical doctors;

2) The personal data provided to effect my registration in the SMARTTM CHIN Course are of my sole responsibility;

3) I am aware that all services will be available only after the payment of the registration, and that it is MY responsibility to monitor communications and changes, as well as to access the site in the participant’s area to participate in the SMARTTM CHIN Course;

4) I also declare that I will have access to the course in a virtual environment offered by SMARTTM INJECTO RSand, in this context, I am aware that the registration is personal, therefore, the reproduction of the respective content, by any means, is forbidden for non-registrants, as well as the recording of images, content and voice, editing, since all its content is the exclusive property of Dr. Thais Sakuma CRM 5868 /MS, protected by copyright law.

5) I am aware that failure to comply with this term may give rise to civil and or criminal liability, in addition to being liable for losses and damages;

6) When confirming the registration in the course through payment, it is agreed that in case of withdrawal from the course by the participant:
• Up to 30 days before the event, 75% of the amount already deposited will be refunded;
• Up to 15 days before the event, 50% of the amount already deposited will be refunded;
• If the event is 14 days or less, there will be no refund of the amount paid;

7) Classes will be available to re-attend only once, for up to 15 days after the date of the live class for reviews and for registrants that for some reason were unable to attend at the day;

8) Classes access will be available through login and password. The transmission system does not allow connection of more than one device;

9) Registration and Payment Methods: After your request to register you will be directed to the Registration Area where you must fill in all the required data, choose the classes for your registration and make the payement;
